Conditions that change the calculus rather than forbidding it Writing it up because I had to work it out twice and would rather nobody else did.

What changes if the standard account of conditions is wrong? I ask because I have been treating it as settled and I noticed this week that I could not say why.

Working through the consequences rather than the evidence, since others here are better placed on the evidence.

Personal or family history of medullary thyroid carcinoma, or multiple endocrine neoplasia type 2, is the clearest contraindication in this class and it is specific rather than general.

I would be interested in a counterexample if anyone has one.

Adding the measurement that the opening post says would settle it.

A rodent toxicology finding underlying a contraindication is worth understanding, because it explains why the contraindication is narrow rather than broad.

Where I part company with the opening post, and it is a narrow parting.

I think the conditions question is answerable and has not been answered, which is a more optimistic position than most of this thread.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

Anyone who has looked at this more carefully, please correct the record.

Where a condition appears as a warning rather than a contraindication, the decision is a clinical one rather than a prohibition.

That is what I would do. It may not be what is correct.

Small methodological point on conditions: repeating a measurement is cheap and resolves most of what is being argued about here at no cost to anyone.

A history of pancreatitis appears in the cautions and the decision around it needs somebody who knows the history rather than a general rule.

Somebody's condition being on a list does not settle the decision, and somebody's condition being absent from one does not either.

The short version is the first sentence; the rest is why.

I would keep conditions and the decision it usually gets used for separate in this thread. They are related and they are not the same question, and merging them is why the last one went badly.

Confirming post #14 from a second method, which matters more than confirming it from a second person.

Conditions looks different depending on whether you are reading the primary literature or the summaries of it, and the difference is not in our favour.

Where I would push back on the conditions consensus is the confidence, not the direction. The direction looks right. The confidence is borrowed.

Where a contraindication is relative rather than absolute, the reasoning usually appears in the assessment report rather than the labelling.

I have been on both sides of the conditions argument in this category within eighteen months, which should tell you how strong the evidence for either side is.

Where labelling has changed, the change is dated and public, and citing the current version is the whole of the discipline.

That is all the detail I have. Someone else will have more.

Worth separating two things that post #21 runs together.

Conditions is a good example of a question where the honest answer is boring and the interesting answers are unsupported. I would go with boring.

What I want from this conditions thread is the list of things that would need to be true for the claim to hold. If we can write that list, we can check it.

Answering the question post #25 raises rather than the one it answers.

Careful with the language on conditions. "Not detected" and "not present" are different findings and the first is a statement about the method.

The arithmetic in post #29 is right; the assumption feeding it is the part to check.

Conditions: I have looked for the primary source twice and failed twice. Either it does not exist or it is somewhere I do not know to look, and I would like to know which.
