Reading a denial letter as a specification for your appeal Writing it up because I had to work it out twice and would rather nobody else did.

Reading back through what has been written here about reading a denial letter, three questions come up every time and only one has ever been answered properly.

Listing all three, with what I think the state of the answer is for each, so the thread can start further along than the last one did.

Nothing here is legal or medical advice, and appeal processes differ enough by jurisdiction and payer that specifics do not transfer.

Coming back to the opening post, because the follow-up matters more than the original answer.

A clinician's supporting letter that addresses the criteria explicitly is worth considerably more than one that describes the patient generally.

That is what the documentation says. What happens in practice is usually close.

Denial letters: the stated reason tells you exactly what to submit next. A denial is not final; it is a specification for an appeal. Reading the reason carefully and responding point-by-point to it works better than general appeals.

Marking that as an opinion rather than a finding.
