Stepping down deliberately, and how to do it without losing progress — setting out what I have, and where I think it stops being reliable.

Practical question with the units stated, because I have seen how quickly these go wrong without them.

I have a 10 mg vial of tirzepatide and I am working to a 2.5 mg step. My syringes are U-100 insulin syringes, 0.3 mL barrel.

I can do the arithmetic and I have done it twice, getting the same answer both times, but I would like someone to check the reasoning rather than the number — specifically whether I have thought about the residual volume correctly, and whether the graduation I am landing on is one a person can actually read.

The opening post put the caveat in the right place and I want to underline it.

Holding a dose indefinitely: the trials did not study indefinite holding at a non-maximum dose. The trials escalated to a target and then held that. What happens if you stay at an intermediate dose for years is not formally studied and extrapolation is the best available reasoning.

I read post #3 twice before replying, because I had assumed the opposite.

Dose and effect are not linear across the studied range for every compound in this class. Assuming a doubled dose gives a doubled effect is the reasoning error behind most disappointment.

Holding at a dose that is working is a legitimate position and it is not what the trial protocols did. The protocols escalated to a target because they were measuring the target dose, not finding each person's minimum.

I would put a moderate confidence on that and no more.

If you are stepping up mainly because the schedule says so rather than because the current dose has stopped doing what you wanted, that is worth noticing before rather than afterwards.

I checked the source rather than the summary, and they differ.

A titration plan written down in advance is easier to stick to and easier to abandon deliberately. An improvised one becomes a series of decisions made on the worst day of each week.

Post #11 is right about the mechanism and I think understates the practical bit.

Stepping down deliberately: the withdrawal trials show that stopping is followed by regain. The step-down literature is thinner. The conservative assumption is that stepping down is followed by some regain, with the magnitude unknown.

I have separated what I observed from what I concluded, which does not always happen.

Going back down a step is not a failure and the trials allowed it. Several protocols permitted a return to the previous dose for tolerability and then a second attempt at the step.

I would put this at better than even and not much better.

Post #18 and I disagree about the size of the effect, not about the direction.

The most common practical error is not the schedule at all — it is losing track of which step you are on after a break, and then resuming at the top rather than re-approaching it.

The uncertainty is in the assumption, not in the calculation.

How the published trials escalated: they used specific step sizes and intervals. The STEP programme used a particular cadence; the SURPASS and SURMOUNT programmes used slightly different ones. Reading them side by side shows the variation is real but small.

Steady state means the plasma concentration is stable from dose to dose. That happens around 4 to 5 half-lives. Before that, the concentration is rising with each dose. Escalating before steady state means escalating on incomplete information about the dose you are on.

Worth checking against a second source before it gets quoted onward.
