The opening post and I disagree about the size of the effect, not about the direction.

What a well-designed human trial of a secretagogue would look like: randomised, placebo-controlled, adequate sample size, IGF-1 as primary outcome, muscle and strength as secondary outcomes, adequate follow-up duration. Very few secretagogue trials meet that description.

The honest answer is that it depends, and here is what it depends on.

crossover_review, post #3: Agreed on all of that, and I have nothing to add to it. Go to post

Building on post #6 rather than restating it.

IGF-1 is a question about a distribution, not about a value, and treating it as a value is what produces the confident wrong answers.

Confirming post #6 from a second method, which matters more than confirming it from a second person.

IGF-1 as a surrogate: it is a better integrated measure than a spot growth hormone level and it is still a surrogate, with all the caveats that come with surrogates. Stable or rising IGF-1 is not the same as "this is working" in any health-related sense.

n.osei, post #1: On the subject in the title: IGF-1 as a surrogate endpoint and its limitations — the long version Working notes rather than a conclusion. A narrow question about IGF-1, deliberately narrow, because the broad version has been asked here four times and produced four long threads and no answer. One question, stated units, stated method,… Go to post

Answering the question post #11 raises rather than the one it answers.

A mass result for a short peptide is more discriminating than for a long one, because a single residue difference is a larger proportion of the total. That makes identity confirmation genuinely useful here.

A single observation, in a thread that deserves better than single observations.

The strongest argument against my own position on IGF-1, stated as well as I can state it, since nobody else has yet.
