Where I have landed on substitution between formats, having got it wrong once in public: the direction is clear, the magnitude is not, and anyone quoting a precise magnitude has borrowed it from somewhere that did not measure it.

Discussing with your clinician is prudent if the gap is weeks or longer. That holds under the stated conditions and I have stated them. Go to post

Worth separating substitution between formats as a question about the compound from substitution between formats as a question about the documentation. They get answered by different people and only one of them is answerable here.

m.rasmussen, post #57: Supply interruptions: official shortage notices are published by regulatory agencies. That is the correct primary source rather than pharmacy rumors or forum discussion. Go to post

I read post #121 twice before replying, because I had assumed the opposite.

Nothing here is medical advice, and decisions about what to do during a supply interruption belong with a prescriber.

The step people skip is the one I have spelled out.

Manufacturer allocation decisions during a shortage are commercial and are rarely explained, which is frustrating and is not evidence of anything.

Old habit: I write down the expected answer before I calculate it.
