Which panel is worth repeating and which is worth ignoring — a second dataset

Asking directly, because I could not find a straight answer: Which panel is worth repeating and which is worth ignoring — a second dataset

Posting a small dataset on panel. It is mine, it is uncontrolled, and the method is stated so it can be discounted appropriately.

What I would like is not agreement but a second dataset collected by someone with no stake in mine. If one exists I would rather read it than argue for this one.

What is documented is narrow. What is inferred from it is broad. The gap between them is where every argument here lives.

Repeat before acting is standard practice for an isolated abnormal result on most analytes, and it is standard for a reason.

Picking up post #3: that is the part I would want checked first.

Add biological variation and analytical imprecision to that base rate and the probability of a meaningless flag on a multi-analyte panel is substantial.

That is the practical version. The rigorous version is longer and says the same thing.

A definition problem is doing most of the work in this panel discussion. Once the term is pinned down I suspect the disagreement mostly goes away and what is left is small.

What I would check first on panel is whether the thing being measured moved or whether the way of measuring it moved. Those look identical in a graph.

Coming back to post #4, because the follow-up matters more than the original answer.

Glycaemic markers integrate over different periods. A point glucose and a longer-term marker disagreeing is not a contradiction; it is two different questions.

It cost nothing to check and would have cost something not to.

Panel is a good example of a question where the honest answer is boring and the interesting answers are unsupported. I would go with boring.

Time of day: some biomarkers vary across the day. Cortisol in the morning differs from cortisol in the evening. Comparing results from different times of day is comparing things that are not the same.

Nothing above should be read as advice about what anyone else should do.

Coming back to post #11, because the follow-up matters more than the original answer.

Where a panel includes a calculated value, that value inherits the imprecision of everything it was calculated from and is usually reported without an interval.

Caveat: everything above assumes the paperwork is what it says it is.

Posting my panel numbers with the method attached so they can be discounted properly. Uncontrolled, unblinded, and collected by someone who wanted a particular answer.

Post #23 put the caveat in the right place and I want to underline it.

I read the earlier replies on panel twice before writing this, because I had assumed the opposite and wanted to be sure I was disagreeing with what was said rather than what I expected.

Taking post #24 at face value and following it one step further.

Counterpoint on panel, offered without confidence: the same observation is consistent with a much duller explanation, and nobody has ruled the dull one out.

Post #26 and I disagree about the size of the effect, not about the direction.

Lipid panel interpretation: total, LDL, HDL, triglycerides all on one panel. Reading them together is more informative than reading one value in isolation. An elevated triglyceride with low HDL is different from triglyceride elevation alone.

Two sources, same conclusion, and I could not rule out that one copied the other.
