Second pass at: Circumference measurements as a poor man's composition estimate — setting out what I have, and where I think it stops being reliable.

Two things I would like separated before anyone answers on Circumference measurements, because they get bundled and then argued about as one thing.

The first is descriptive: what has actually been observed, by whom, and how. The second is causal: why. I am asking about the first only.

Marking my uncertainty on Circumference measurements explicitly. I am confident about the direction, much less confident about the size, and not confident at all that it generalises past the case in the first post.

Percentage body fat is a ratio, so it changes when either component changes. A rising percentage during weight loss is arithmetic before it is physiology.

Resting metabolic rate: body composition affects metabolic rate. As fat mass decreases and lean mass is maintained, metabolic rate is usually maintained better than with simple weight loss alone.

The general answer and the answer for your case may diverge here.

Reading back through the Circumference measurements threads from last year, the same three questions come up every time and only one of them has ever been answered properly. That seems like a documentation gap rather than a knowledge gap.

Post #3 put the caveat in the right place and I want to underline it.

Comparing a body composition result against a population reference is a different exercise from tracking your own change, and the second is what most people want.

Old habit: I write down the expected answer before I calculate it.

Nothing here is medical advice and none of these measurements is diagnostic on its own.

On post #7 — agreed on the reasoning, with one qualification.

Adding a reference point for Circumference measurements. Mine is a single case, collected without controls, and I am posting the method alongside it so it can be discounted appropriately.

Changes during titration: measuring body composition during active titration is less useful than measuring after dose is stable. Titration causes fluid shifts that confound body composition.

Not a conclusion. A place to stand while looking for one.

Coming back to post #10, because the follow-up matters more than the original answer.

A note on scope: what I am saying about Circumference measurements applies to the case in the first post and I would not extend it further without checking.

Adding the measurement that post #13 says would settle it.

The most useful protocol is the one you will actually repeat identically. An imperfect measurement repeated consistently beats a better one repeated carelessly.

Two people can read the same figure differently here and both be reasonable.

Circumference measurements at marked sites, taken consistently, track composition change better than most people expect for the cost.

Noting that the question and the thing people usually mean by it are different.

Photographs under fixed lighting and posture are a genuine record and are the hardest of these to standardise.

Written quickly, so the reasoning may be tighter than the wording.

The arithmetic in post #16 is right; the assumption feeding it is the part to check.

Speaking only to Circumference measurements as I have actually seen it, rather than as it is usually described: the effect is real, it is smaller than the thread suggests, and the variance between people is larger than the effect.

Answering the question post #19 raises rather than the one it answers.

Neither method is capable of detecting the month-to-month changes that people report detecting month to month.

The bit of Circumference measurements that nobody enjoys is that the answer changes depending on what you are trying to decide with it. Say what the decision is and the thread will converge.

Report the method with the number, always. A composition figure without the method is not comparable to anything, including your own previous figure.

Written from notes rather than memory, which is why the numbers are specific.

Narrowing post #23, because the general version has more than one answer.

DXA (dual-energy X-ray absorptiometry): the standard for body composition in research. Precision is roughly ±1 kg for lean mass. Two scans six months apart on the same machine with the same technician give you useful information. Two scans six weeks apart on different machines tell you about the machines.

The interesting part of this is the exception, and I do not understand the exception.

Worth separating two things that post #23 runs together.

Bone mineral density changes with substantial weight loss are measurable and slow, and are one of the better reasons to have a baseline scan.

If that reads as pedantic, it is, and it has saved me twice.

This follows post #23 rather than contradicting it.

Maintenance phase: tracking body composition during maintenance (weight stable) is different from tracking during active loss. Changes are smaller and precision becomes more important.

A modest claim, modestly supported.

Worth stating the null on Circumference measurements before we explain it: the observation may be nothing. That possibility deserves a sentence and usually does not get one.
