What a good consultation looks like from the other side — that is the question, and I have not found it answered plainly anywhere I have looked.
Something about good consultation does not reconcile and I would like a second pair of eyes before I decide which half is wrong.
Two sources, both reputable, giving figures that cannot both be right unless they are measuring different quantities. My suspicion is that they are, and I cannot see how.
Good consultation would be much easier to settle if anyone reported the denominator. Almost nobody reports the denominator.

Having read the whole good consultation thread before replying: the question in the first post has not actually been answered yet, and three of us have answered a nearby one instead.
On post #9 — agreed on the reasoning, with one qualification.
Written summaries after an appointment are available in many systems and are worth asking for, because recollection of a consultation is unreliable.
On reflection I would soften that slightly.
That reframing is the whole thing. The facts I already had.
I would call the community position on good consultation likely rather than established, and I would be comfortable defending that hedge.
I had written a reply contradicting post #53 and deleted it. Here is what survived.
Small methodological point on good consultation: repeating a measurement is cheap and resolves most of what is being argued about here at no cost to anyone.
Summarising the good consultation thread so far, since it is long and the answer is buried: the first reply has the method, the fourth has the correction to it, and the rest is people agreeing at length.
