The arithmetic in the opening post is right; the assumption feeding it is the part to check. Visible particulate is a strong signal in the other direction: it is a reason to stop and ask rather than to filter and proceed. Posting it because the silence on this was starting to look like agreement.

A parcel that arrived warm: a decision process, not a verdict Writing it up because I had to work it out twice and would rather nobody else did.

Asking about the failure mode rather than the procedure, because the procedure is documented and the failure mode is not.

I know what I am supposed to do. What I do not know is what it looks like when it has gone wrong, and by the time something is obviously wrong it is usually too late to learn from it.

So: what does the early version of this mistake look like, and what would you check first?

Temperature excursions in transit are common and are not all equal. A few hours warm is a different event from three days warm, and neither is visible in the vial on arrival.

A single observation, in a thread that deserves better than single observations.
