How to ask for a dose change without antagonising anyone — what changed since — that is the question, and I have not found it answered plainly anywhere I have looked.

A question about how to describe something to a clinician rather than about the thing itself.

I have a set of observations, dated, with what I was doing at the time. What I do not have is a sense of which parts are worth an appointment's limited minutes.

How would you prioritise this if it were yours to present?

Answering the question the opening post raises rather than the one it answers.

A question phrased as "what would you want to see before considering X" is answerable and is not a request for X.

It is a small point and it changes the answer, which is an awkward combination.

Discussing side effects: be specific. "Nausea" is less useful than "I have nausea worst 24 to 36 hours after injection, severity 2 to 3 out of 4, stable over weeks". That description helps determine whether to adjust dose or dose timing.

I would put a moderate confidence on that and no more.

That is what I would do. It may not be what is correct.

How to ask for what you need: rather than "should I take this", frame it as "I understand the evidence for this shows X benefit in people with my condition; would it be suitable for my specific situation?" That invites clinical reasoning rather than yes/no.

I looked this up rather than remembered it, which is the right order.

Questions about dose: asking "how will we determine whether this dose is working for me" gets at the monitoring plan. Asking "can we start lower" or "can we escalate slower" opens that conversation.
