Orforglipron as a non-peptide: what changes when the molecule is small

On the subject in the title: Orforglipron as a non-peptide: what changes when the molecule is small Working notes rather than a conclusion.

What changes if the standard account of orforglipron is wrong? I ask because I have been treating it as settled and I noticed this week that I could not say why.

Working through the consequences rather than the evidence, since others here are better placed on the evidence.

A definition problem is doing most of the work in this orforglipron discussion. Once the term is pinned down I suspect the disagreement mostly goes away and what is left is small.

Filing a mild objection to the consensus on orforglipron. Mild because I might be wrong; an objection because nobody has addressed the case that does not fit.

Why an oral formulation is a formulation achievement: the molecule is the same but the tablet is novel. Getting a peptide across the gastric epithelium at usable bioavailability is a chemistry problem, not a dose problem.

I would call that likely rather than established.

Timing consistency matters more for oral dosing than for weekly injection, because absorption depends on the state of the stomach and the state of the stomach varies through the day.

Picking up post #7: that is the part I would want checked first.

Adding what did not work for me on orforglipron, since the failures never get written up and they are half the useful information.

On post #8 — agreed on the reasoning, with one qualification.

Orforglipron is a small molecule rather than a peptide, which changes almost everything about how it is made, stored and analysed. Comparing it to oral semaglutide as though they were the same pharmaceutical problem is a category error.

Noting that the question and the thing people usually mean by it are different.

Storage for a small molecule is a different problem from storage for a peptide: generally more robust, generally less temperature-sensitive, and generally more affected by humidity.

Worth separating two things that post #11 runs together.

The claim about orforglipron upthread is stronger than its source supports. I have read the source. The source says "associated with" and the post says "causes".

My understanding of orforglipron is a few years old and may have been superseded. If it has been, I would genuinely like to know rather than keep repeating it.

Timing consistency matters more for oral dosing than for weekly injection, because absorption depends on the state of the stomach and the state of the stomach varies through the day.

I would rather say I do not know than round it up to an answer.

The reason orforglipron is hard to answer is that the obvious measurement and the relevant quantity are not the same thing, and substituting one for the other is silent.

Answering the question post #15 raises rather than the one it answers.

PIONEER 6 was a cardiovascular safety trial for oral semaglutide, not an efficacy trial. Non-inferiority for safety was demonstrated. The point estimates favoured the drug but the trial was not designed to establish benefit.

Where I would look next, rather than where I would stop.

Orforglipron is a small molecule, not a peptide. That changes almost everything: no absorption enhancer required, no fasting window, chemical synthesis instead of peptide synthesis, different analytical methods entirely. Data from peptide agonists does not transfer.

The strength of my opinion here exceeds the strength of my evidence.

Taking post #23 at face value and following it one step further.

A methods point on orforglipron rather than a substantive one: if the comparison is not like for like, the difference you are measuring is the difference in method.

I read post #26 twice before replying, because I had assumed the opposite.

A small molecule can be characterised by nuclear magnetic resonance and by mass spectrometry against a reference standard, which is a considerably stronger identity claim than a peptide chromatogram usually supports.

If this contradicts something upthread, the upthread version may well be the better one.

Marking my uncertainty on orforglipron explicitly. I am confident about the direction, much less confident about the size, and not confident at all that it generalises past the case in the first post.
