I am confident about the direction and much less about the magnitude.

Decimal-point error is a question about a distribution, not about a value, and treating it as a value is what produces the confident wrong answers.

The number is defensible. The precision I gave it is not.

Adding a reference point for decimal-point error. Mine is a single case, collected without controls, and I am posting the method alongside it so it can be discounted appropriately.

The honest answer on decimal-point error is that it depends, and the useful part is the list of what it depends on. Four items, in rough order of how much they matter.

Most people get the first two right and then argue about the fourth.

I read post #70 twice before replying, because I had assumed the opposite.

Write the reconstitution date and the concentration on the vial. Not on a note, on the vial. Every account here of a dosing error involving the wrong concentration involves a vial with nothing written on it.

I would not lead a decision with this, but I would not ignore it either.

Worth separating decimal-point error as a question about the compound from decimal-point error as a question about the documentation. They get answered by different people and only one of them is answerable here.

Where I have landed on decimal-point error, having got it wrong once in public: the direction is clear, the magnitude is not, and anyone quoting a precise magnitude has borrowed it from somewhere that did not measure it.

Small correction to my own earlier position on decimal-point error. I had the units the wrong way round, which changes the conclusion by an order of magnitude and therefore changes it entirely.

Decimal-point error: I would want to see the raw numbers rather than the summary before agreeing. Summaries lose exactly the information that would settle this.

Where I part company with post #88, and it is a narrow parting.

On decimal-point error, I would rather understate and be corrected upward than overstate and be quoted. That is a house style here and it is a good one.
