Posting a small dataset on SURMOUNT-4 and continuation versus withdrawal. It is mine, it is uncontrolled, and the method is stated so it can be discounted appropriately.

What I would like is not agreement but a second dataset collected by someone with no stake in mine. If one exists I would rather read it than argue for this one.

The documentation on SURMOUNT-4 and continuation versus withdrawal is better than this thread and I say that as someone who has posted in the thread.

SURMOUNT-1 (N Engl J Med 2022): Tirzepatide obesity trial. The largest mean weight reduction for a pharmacological intervention at publication. Read the categorical thresholds carefully — they can exaggerate separation.

This is the sort of thing the wiki should carry and currently does not.

Reading order that works for these sessions: registry entry, methods, baseline table, primary result, then abstract last. Reading the abstract first anchors everything that follows.

Reading it back, the second half matters more than the first.

On post #2 — agreed on the reasoning, with one qualification.

My understanding of SURMOUNT-4 and continuation versus withdrawal is a few years old and may have been superseded. If it has been, I would genuinely like to know rather than keep repeating it.

Confirming post #4 from a second method, which matters more than confirming it from a second person.

The baseline characteristics table is the single most useful page for the questions that get asked here, because it tells you who the result applies to.

Where the group cannot agree, record the disagreement rather than resolving it by seniority. The recorded disagreement is more honest and more useful later.

That is the practical version. The rigorous version is longer and says the same thing.

Practical note on SURMOUNT-4 and continuation versus withdrawal: write down what you expect before you look. The number of times I have found what I went looking for is higher than chance would allow.

Narrowing post #9, because the general version has more than one answer.

I disagree with the framing of SURMOUNT-4 and continuation versus withdrawal above, and I think it is a substantive disagreement rather than a terminological one. Setting out why, so it can be checked.

The reasoning depends on an assumption that is doing a lot of work and is never stated. If the assumption holds, the conclusion follows. I do not think it holds generally.

The figures usually contain the finding and the text usually contains the interpretation. Separating them for the first twenty minutes is a discipline worth keeping.

That is all the detail I have. Someone else will have more.

Two things can be true about SURMOUNT-4 and continuation versus withdrawal at once: the mechanism is plausible and the evidence for the size of the effect is thin. Most of the argument here is people defending the first against attacks on the second.

The arithmetic in post #14 is right; the assumption feeding it is the part to check.

An hour is enough for one paper read properly and not enough for two read partly. The temptation is always to add the second.

This is where my knowledge stops and I would rather mark the edge than blur it.

Where I have landed on SURMOUNT-4 and continuation versus withdrawal, having got it wrong once in public: the direction is clear, the magnitude is not, and anyone quoting a precise magnitude has borrowed it from somewhere that did not measure it.

The most common failure mode is spending fifty minutes on the effect size and ten on the population. Reversing that ratio would improve most sessions.

Small correction to my own earlier position on SURMOUNT-4 and continuation versus withdrawal. I had the units the wrong way round, which changes the conclusion by an order of magnitude and therefore changes it entirely.

Nobody has said the unglamorous part of SURMOUNT-4 and continuation versus withdrawal yet, so: most of the variation is explained by things that are boring to write about and easy to check.

My experience of SURMOUNT-4 and continuation versus withdrawal contradicts the reply above. I am posting it as a data point rather than as a refutation, because one person's experience is exactly that.
